• All form fields are required unless otherwise denoted as optional
• Please take care when filling in the form, as errors may require verification

There are two forms:

• Child Form. An adult fills in this form on behalf of their child
• Adult Form. For players 18 years of age and older

Under 6 - Born after 2003	$50.00
Under 8 - Born in 2002 and 2003	$50.00
Under 10 - Born in 2000 and 2001	$100.00
Under 12 - Born in 1998 and 1999	$115.00
Under 14 - Born in 1996 and 1997	$115.00
Under 16 - Born in 1994 and 1995	$115.00
Under 18 - Born in 1992 and 1993	$115.00
Adult - Born before 1992	$115.00

You do not need to submit payment with your registration. Money is collected at a later date.
